Home blood pressure monitoring and drug self-titration in hypertension: patients' and general practitioners' perceptions. AMTITEA qualitative study

Context. The combination of home blood pressure monitoring (HBPM) and drug self-titration (DST) improves blood pressure control by empowering patients. Still little used in France, the opinions of French patients and general practitioners (GPs) on this intervention were not known.Objective. To explore patients' and GPs' barriers and levers to combine HBPM and DST in the management of hypertension.Method. Qualitative study using a global inductive analysis approach. Semi-directed individual interviews of GPs and patients with hypertension using HBPM in Auvergne. Categorization of raw data and blind parallel coding developed a conceptual network related to research objective.Results. Nine GPs and 14 patients were involved in the study. For the patients, the obstacles to DST were their limited medical knowledge and the risk of medication misuse. The expected benefits were a lower number of medical consultations and better daily blood pressure monitoring. The higher socio-professional categories and patients with high cardiovascular risk seemed to be more motivated. For GPs, the obstacles identified were the lack of time to spend on this education, and the necessity to supervise patients. According to the GPs, empowerment was not a major request of the patients and the intervention targeted a restricted patient profile. The expected benefits were patient involvement, facilitation of communication, and help in better understanding the disease. Young women GPs seemed to be the most convinced.Conclusion. Both patients and GPs have mixed views regarding DST. This explorative work will make possible the adaptation of an interventional study applying HBPM associated with DST in French hypertensive patients.
